The damage often does not show up on day one. It shows up months later as creeping loss or a sudden outage.

1. Exceeding the Maximum Pulling Tension

Every cable has a maximum tension rating. Pull harder than that and you stretch the fibre inside, which may not break immediately but leaves micro-damage that degrades over time.

This happens most often when a crew uses a winch without a tension monitor, or when they pull the whole drum length in one go around multiple bends. Use a breakaway swivel, pull in stages with intermediate assist points, and never attach the pulling rope directly to the fibres — use the strength members or a proper pulling grip.

2. Violating the Bend Radius

Fiber tolerates bending, but only down to a specified radius. Tighter than that and light escapes the core — the loss shows up immediately in testing, or worse, intermittently under temperature changes.

The common failure points are not the main route. They are inside patch panels, joint enclosures, and at wall entries, where slack gets coiled too tightly to fit the space. Two rules: never coil fibre tighter than the specified minimum radius, and never use cable ties pulled tight on fibre cable.

3. Poor Cleaving Before Splicing

A fusion splice is only as good as the cleave. If the cleave angle is off, the fibres will not fuse cleanly and you get a high-loss joint that the splicer may still report as acceptable.

Cleaver blades wear. Rotate and replace them on schedule, keep the cleaver clean, and if splice loss suddenly rises across several joints, suspect the cleaver before blaming the cable.

4. Dirty Connector End Faces

This is the most common cause of unexplained loss in the entire industry, and the most preventable. A single particle of dust on a connector end face can cause significant loss — and when mated under pressure, it can permanently scratch the ferrule.

Clean every connector before every mating. Every time, not just when there is a problem. Keep dust caps on unused ports and on patch cords in storage.

5. Inadequate Slack at Joints and Terminations

Leaving no slack means that any future repair requires a new joint, and any settling of the cable puts tension directly on the termination.

Leave sufficient service loops at joint enclosures, at building entries, and at patch panels — enough that a technician can bring the enclosure to a comfortable working position and re-splice if needed. This costs a few extra metres of cable and saves an entire repair job later.

6. Improper Sealing of Outdoor Enclosures

Water is the enemy of every outdoor joint. Moisture inside a closure leads to corrosion, freezing damage in cold regions, and eventually failed splices.

Follow the closure manufacturer's sealing procedure exactly — the correct gaskets, the correct torque, the correct grommet size for the cable diameter. Do not improvise with tape. And do not leave unused ports open.

7. Skipping Proper Testing and Documentation

Testing is not just about proving the link works today. An OTDR trace taken at handover is a baseline — when a fault appears in two years, comparing against that baseline tells you exactly what changed and where.

Document fibre counts, splice locations, route diagrams, and test results. The next person to touch this network will not be you, and undocumented networks get damaged by people guessing.

Every one of these mistakes has the same shape: the link passes initial testing, so nobody notices, and the cost appears later as a callout, an outage, or a re-dig.
